The 28-year-old joined the Reds, who won the Premier League last season, in 2016 after a successful trial within the academy.
Across the subsequent nine years at the club, Phillips made 29 senior appearances with his debut coming against Merseyside rivals Everton in a 2020 Emirates FA Cup tie.
After a loan spell at Stuttgart in Germany, which was interrupted by his return to Anfield for his senior debut in Red, he joined AFC Bournemouth on a temporary basis.
The centre-back helped the Cherries to secure promotion into the Premier League, before being loaned to Scottish giants Celtic, Cardiff City, and most recently Derby County.
In the 2020/21 season, Phillips played 20 times for an injury-hit Liverpool, as he became an unlikely, yet important, part of a campaign that saw them qualify for the UEFA Champions League.
West Brom's new recruit made 33 appearances for Derby last term, 32 of which came in the Sky Bet Championship, as the Rams stayed clear of relegation in their return to the second tier.
Phillips scored once and provided three assists across the season, following on from an 18-appearance spell at Cardiff the season prior.
While the experienced Championship defender officially joins West Brom on an undisclosed fee, journalist Paul Joyce has reported that the deal is worth £3 million including potential add-ons.
Albion finished ninth last season, four points behind Bristol City in the final play-off position.
