The visitors thought they'd taken the lead inside the opening 10 minutes as Tyrese Campbell headed them in front, but the goal was ruled out for offside after intervention from the linesman.
Chris Wilder's side eventually secured an advantage before half time as Bristol City's Rob Dickie was sent off for a challenge on Kieffer Moore and Harrison Burrows slotted home the resulting penalty.
Burrows then turned from scorer to provider in the second half as he set up teammate Andre Brooks with 15 minutes left to play.
Bristol City's task then became even harder as substitute Callum O'Hare scored the visitor's third and final goal of the evening, meaning a sensational comeback will be required in the second leg at Bramall Lane next week if the Robins are to have any chance of keeping their promotion hopes on the line.
For the Blades, after narrowly missing out on automatic promotion, they are now firmly on track to reach the play-off final at Wembley Stadium, with Sunderland and Coventry City contesting the other semi-final.
