The Briton fell out of favour under Pep Guardiola last season and requested a transfer during the January transfer window.
He could not find a permanent switch, though, instead joining AC Milan on a short-term loan, where his fortunes did not improve as the Rossoneri struggled to an eighth-place finish in Serie A.
But Walker, who has 96 England caps, has now found a new home in the Premier League, with Scott Parker’s Burnley emerging as a surprise destination for the 35-year-old.
According to reports in The Athletic, he is already undergoing medical testing.
A two-year deal has been proposed, and the transfer will be worth as much as £5million should add-ons be achieved.
It is a beneficial deal for City, too, who are set to save as much as £10m per season as they are set to offload one of their highest earners.
The move will prompt a reunion for Walker and Parker after the pair were Tottenham teammates between 2011 and 2013.
Parker’s side will benefit from Walker’s wealth of top-flight experience as they look to survive in the Premier League, where the full-back has played 410 matches.
Burnley finished second in the Championship last season. They were equal with champions Leeds at 100 points apiece, but fell short on goal difference.
They return to the Premier League after spending a solitary season in the second tier. They finished 19th in the Premier League during the 2023-24 campaign.
Walker, a Sheffield United academy graduate, is a six-time Premier League champion after spending eight years with City. He has also lifted the Champions League, three League Cups, two FA Cups, and the Club World Cup.
His last City appearance came in a 4-1 win over West Ham in January.
